Healthy Steps 2018 - 2019
The 2018-2019 Healthy Steps incentive program began on July 1, 2018 and ends May 31, 2019. Throughout the year, you* will have opportunities to earn points by participating in wellness programs, health screenings, and attending wellness classes. In May 2018, you will need to complete our on-site health screening in order to receive any of the incentives in the program.

You are encouraged to complete a preventive routine physical exam with your primary care physician between July 1, 2018 and May 31, 2019 to receive 100 points.
Have your physician complete and sign the linked Reporting Form. The Reporting Form must be submitted to Jennifer Geist by May 31, 2019.
Our Chronic Condition Management Program is designed for those individuals with uncontrolled diabetes, blood pressure, or cholesterol.
Chronic Disease Self-Management Workshop
Chronic illness can have a major impact on your lifestyle and unfortunately, most of us will experience at least one chronic illness during our lives. Participate in this interactive 6-week workshop and learn about the tools, resources, and strategies you can use to positively manage your condition so you can lead a healthy, productive life. In this program you will develop the skills you need to:
- Manage your illness
- Carry out your normal activities
- Deal with the changing emotions brought on by chronic illness
- Achieve the highest possible capability and enjoyment with your life

If you use tobacco products, you are encouraged to enroll in a Tobacco Cessation Program to receive 100 points
Have your Tobacco Cessation Program provider complete the linked Reporting Form. The Reporting Form needs to be submitted to Jennifer Geist by May 31, 2019.
Earn 25 points for each wellness class attended. The maximum points you can earn is 100 (4 classes). For Huntsville-based employees, only 2 on-line classes may count toward points.
On-line wellness classes are available through CIGNA. See more information about this in the "More Details" section below or by clicking on the "Classes" tab above.
After you view a class, please complete the linked Evaluation Form and email it to Jennifer.Geist@Teledyne.com. All wellness class evaluation forms must be submitted to Jennifer Geist by May 31, 2019.
This is a confidential wellness program. Only Jennifer Geist, Wellness Manager has access to your confidential data. HIPAA requirements for the storage and security of protected health information will be fully met.
If it is unreasonably difficult for you to achieve the standards for the reward under this program due to a medical condition, or if it is medically inadvisable for you to attempt to achieve the standards for the reward under this program, call Jennifer Geist, TBE Wellness Manager, and she will work with you confidentially to develop another way to qualify for the reward.
*The Healthy Steps Program rewards are available to all full-time and special part-time employees. The rewards are considered taxable income and will be included on your Form W-2. The taxes will be reflected on your pay stub.
On-line wellness classes are available through CIGNA. To access the online classes, go to the "Classes" section of the Wellness Website.
